In fact, lots of tranparency, effects and colour management! But for a new publisher my book had to be scaled to a new series format size. As they needed PDF x1/a before (so flattened and CMYK managed) going through Postscript was oke. Thetr is no problem at all with transparency and postscript through Adobe PDF 9 Acrobat driver, and all was managed to 320% ink coverage when distilling with the right profile. All printed without any problem, all transparency examples (including knock out groups in all its variations!) in the book printed beautifuly.
That said: I really would see a way to scale directly to PDF output from within InDesign... !
